Policy Agency personnel will prepare written documentation reflecting the reason why the visit was missed. Procedure 1. A missed note will be prepared and signed by the appropriate discipline that missed the visit. 2. The note will document the specific reason why the visit was missed and with whom agency personnel communicated with. 3.
