MDT Home Health Care Agency, Inc. WORK RELATED INJURIES POLICY: MDT Home Health Care Agency, Inc. will ensure that all Employees who suffer work-related injuries or illnesses receive appropriate attention. MDT Home Health Care Agency, Inc. will comply with all regulations pertaining to work-related injuries. MDT Home Health Care Agency, Inc. provides a comprehensive Workers’ Compensation Insurance program at no cost to Employees. This program covers any injury or illness sustained in the course of employment that requires medical, surgical or hospital treatment. Subject to applicable legal requirements, Workers’ Compensations Insurance provides benefits after a short waiting period or immediately, if the Employee is hospitalized. Apparent trends or widespread problems will be brought to the attention of the QI committee for analysis and study. PROCEDURE: Employees who sustain work related injuries or illnesses should inform the Supervisor at the time of injury. For Employees requiring outside medical treatment, an Employee Incident Report should be completed and forwarded immediately to the Human Resource Department. Within 24 hours, the Employee’s immediate supervisor or Human Resource Designee will complete a First Report Of Injury or Illness and send it to the insurance company. The Supervisor or designee : See that the injured person receives prompt medical attention. · Review the incident with the Employee as soon after the incident as possible. · Forward to the Human Resource Department all work related injuries or illnesses for investigation and recording. · Notify the Director and/or VP of Operations. Medical attention may be obtained from the nearest hospital, the employee’s private physician or the agency-designated physician. If medical attention not desired by the employee, he or she must sign a Worker’s Compensation Waiver form. The agency must notify the emergency room or private physician that this is a worker’s compensation claim and that additional information may be obtained by contacting the agency. Medical follow up for occupation exposure incidents is in accordance with Occupational Safety and Health Administration (OSHA) regulations. If the injury or illness meets the definition of a recordable injury by OSHA definition, it must be entered on the OSHA Log form 200 within six (6) working days. Work related injuries resulting in lost time may require a medical leave of absence. If a work related injury qualifies as a “serious medical condition” under the Family and Medical Leave Act (FMLA), the Human Resource Department should advise the Employee of his/her right to request leave under FMLA. The Company will make a diligent effort to provide modified duty for all Employees who have been injured in the line of duty and who have been released by the treating Physician with a restriction.
