MDT Home Health Care Agency, Inc. RETENTION OF PERSONNEL RECORDS POLICY: Personnel records shall be retained for the duration of employment in a confidential manner with access only by authorized staff, and for such additional time as may be required for historical reference and/or legal requirements. MANAGEMENT OF PERSONNEL RECORD PROCEDURE: Personnel records shall be kept a minimum of three (3) years after termination of Employment, must be stored in a locked cabinet, maintained in a confidential manner with access only by authorized staff within the Human Resources Department under the direction of the Agency’s Administrator. All Personnel Records are maintained up to date for the Human Resources Department for that reason must be weekly reviewed, taking in care all required documentation, including but not limited to: Application, Referrals, Contract, Job Descriptions, Continue Education credits, internal In-services, orientation and/or training, License and verification, Health and Criminal records (that must bemaintained inside the Staff record in a confidential enveloped), Probation and Annual Evaluation, competencies, etc. Records must be retained by the time periods set forth below or the amount of time required by applicable state statute. Records relevant to a legal action are not to be destroyed without the approval of the President of the Board. Advertisements for employment shall be kept a minimum of one (1) year. Applications for employment shall be kept a minimum of one (1) year. Attendance records shall be kept a minimum of three (3) years. Payroll records and time sheets shall be kept a minimum of three (3) years. Job descriptions shall be kept a minimum of two (2) years or until superseded. Safety or injury frequency reports (including OSHA records) shall be kept a minimum of ten (10) years. Exposure reports should be kept for a minimum of thirty (30) years.
