MDT Home Health Care Agency, Inc. EMPLOYEE DATABASE POLICY: The Human Resource Department is responsible for data entry and maintenance of the employee database. This database includes employee demographics, employee pay rates, deduction information, and primary work centers. A Personnel Action Form (PAF) is required to add new employees, terminate, or edit this information. Payroll staff does not have access or authority to change this information. All information is confidential. PROCEDURE: · A Personnel Action Form (PAF) must be completed when a new employee is hired, employee information is changed, or an employee is terminated. The Human Resource Designee completes the form on all new employees. Director of Nursing, Clinical Manager completes the form for changes and terminations. · The Human Resource designee is responsible for data entry of employee information into the computer system. · The Human Resource Designee forwards the personnel action forms to Payroll after data entry into the Human Resources management information system. · The Director of Nursing, Clinical Manager or Administrator must complete a Personnel Action Form on employees who have terminated indicating leave balances. This form is then forwarded to Payroll staff for processing of the final paycheck. All work and documentation of visits must be completed before the leave balance checks are distributed.
