MDT Home Health Care Agency, Inc. INTRODUCTORY PERIOD POLICY: The introductory/probationary period is intended to give new Employees the opportunity to demonstrate their ability to achieve a satisfactory level of performance and to determine whether the new position meets their expectations. MDT Home Health Care Agency, Inc. uses this introductory period to evaluate Employee capabilities, work habits, and overall performance. The introductory period will be for ninety (90) days from the day of hire. PROCEDURE: · All new and rehired Employees work on an introductory basis for the first ninety (90) calendar days after the date of hire. · Employees placed in a new position shall work on an introductory basis for the first ninety (90) calendar days after the date of status change. · Any absence greater than one week will automatically extend an introductory period by the length of the absence. · If MDT Home Health Care Agency, Inc. determines that the designated introductory period does not allow sufficient time to thoroughly evaluate the Employee’s performance, the introductory period may be extended for a period of thirty (30) calendar days. The Human Resource Department should be contacted prior to the extension of the introductory period. · Employees will be evaluated within two weeks of the end of their introductory period. Prior to the completion of the Job Specific Evaluation, the employee should complete the self- evaluation and turn in to their supervisor. Both the self-evaluation and the Job Specific Evaluation will be reviewed with the employee at the end of the introductory period. Merit increases are awarded on an annual basis. In addition, the supervisor may utilize the Employee Review Form to periodically review the employee’s progress and performance. · Employees, with or without a performance evaluation, will become regular Employees at the end of the introductory period. This introductory period does not change the employment-at- will nature of employment by MDT Home Health Care Agency, Inc. · During the introductory period Employees are eligible for benefits as specified by policy and as required by law.
