MDT Home Health Care Agency, Inc. PAY PERIOD References: Fair Labor Standards Act: Title 29, Part 516, Part 758 POLICY: It is the policy MDT Home Health Care Agency, Inc. to assure Employees receive their paychecks in a timely manner in accordance with the established pay period schedule and as set forth by the Fair Labor Standards Act (FLSA). PROCEDURE: · MDT Home Health Care Agency, Inc. pay periods are weekly beginning at 12:00 A.M. Sunday and ending one week later at 11:59 PM on Saturday. · Overtime calculations are based on actual time worked in excess of 40 hours. · Paychecks are available on agency designated day. · Terminated Employees will receive their final paycheck per the regularly scheduled payday following the pay period ending in which the last day was worked unless otherwise specified by law. · Paychecks may be released to the Employee at the Corporate office, mailed directly to the Employee’s home address or direct deposited. · An Employee may request direct deposit by submitting a blank voided check to the Human Resource Department. · Employees who lose their paycheck shall immediately notify the Payroll Department. · It is the Employee’s responsibility to notify payroll or Human Resources of all changes to personal data including address change, name change, exemptions, etc. and to ensure all benefit/payroll information is correct. · All benefit, status or wage increase changes will be effective the first day of the next pay period following the change. All changes will be effected by the completion of a Personnel Action Form (PAF). . Some changes will be made effective as soon as possible as dictated by program procedures such as IRA and health and dental coverage. Home Health Agency. - - Personnel/Operations Policies B-124
