TIMESHEETS/DVR’S Reference: Fair Labor Standards Act: Title 29, Part 516, Part 758 POLICY: All MDT Home Health Care Agency, Inc. salaried exempt and hourly Employees must accurately record time worked on a payroll time sheet. Employees must sign the time record certifying that the time recorded is true and accurate. MDT Home Health Care Agency, Inc. will maintain a work record to accurately compensate Employees and to meet Federal and State record keeping requirements. PROCEDURE: Employees must accurately record all hours worked. Hours worked is all the time an Employee is performing his/her job responsibilities. Hourly Employees shall be paid for all hours worked. Salaried-Exempt Employees may be paid a full day for partial days worked not to exceed 40 hours. Working off the clock or not recording time worked is strictly prohibited Time records must be completed in ink, preferably black ink. Any fraudulent or intentional entry of incorrect information on the timesheet or DVRmay result in immediate termination of employment. Time records are property of the Company. Corrections of time records shall be done by a Supervisor or designee and be properly supported with documentation; initialed by the supervisor, or designee and the Employee, and dated. The following procedure should be utilized: Time corrections must only be recorded in ink. Actual time must be recorded by the Employee. If a correction is completed on an original record, a line should be drawn through the incorrect time, corrected and initialed by employee and supervisor. All adjustments should be supported by a brief explanation. Corrections must be initialed and dated by both the Supervisor or designee and the Employee. Corrections related to mathematical errors by the employee may be corrected by the supervisor without employee initials. PAID TIME OFF (PTO): PTO must be recorded on the time sheet in the designated area in order to be paid for time off. PTO will not be paid from the PTO request form. Visiting employees who normally use a DVR to record workday will need to date the DVR and write “8 hours PTO” to account for paid time off during work week. Each employee should have a DVR for each day worked unless off for full week, then the employee may write on one DVR “40 hours PTO” and complete with dates. Jury Duty hours should be indicated on the time record. Documentation of the jury duty must be placed in the personnel file. The Employee will receive their regular wages or salary for any scheduled one (1) day spent on jury duty, less compensation received from the court. · Mandatory work related meetings or seminars attended during normal working hours are considered hours worked. Timesheets and DVR’s will be reviewed and approved by the employee’s supervisor prior to submitting to Payroll. Timesheets and DVR’s not received by Payroll by 9:00 am on Mondays will not be processed until the following pay period. Repeated failure of an Employee to submit pay records timely may result in disciplinary action and/or Performance Improvement Plan. Home Health Agency. - - Personnel/Operations Policies B-125
