MDT Home Health Care Agency, Inc. of the teaching plan is to provide continuity of care for the patient. One way to accomplish this is through careful documentation. The documentation should reveal what teaching was planned, accomplished and what the patient has learned. SAFETY: Educate family members and patient on caution regarding: a) Restrained patients; c) Blind/deaf and mute patients. b) Seizure patients; and also, check safety equipments in the home to be sure they are functional.
