HOME CARE INFUSION THERAPY POLICY AND PROCEDURE Purpose To promote a safe, efficient and cost-effective environment that promotes the quality of life for the patient in the home setting. To ensure that all state, federal and Professional standards are met to safeguard the patient, the nurse, and the agency. Policy Intravenous therapy performed in a safe and efficient manner. The agency will be accountable and responsible for the appropriateness of care for positive outcome. Procedure 1. A physician's order shall be written specifically for home intravenous therapy. 2. Home intravenous therapy shall be provided by a licensed nurse, AlI Licensed Practical Nurses must be trained and evaluated for competency demonstrated by a Registered Nurse prior to assignment. All nurses will be CPR certified in order to perform this task. 3. The nurse shall have the knowledge and demonstrated competency in interpreting and implementing the physician written order. 4. A Registered Nurse shall be available through all clinical aspects of intravenous administration and on-call 24 hours per day. The Registered Nurse will provide the care coordination and integration of all support services in order to assure continuity of care. 5. The patient and/or caregiver will be assessed to reliability, willingness and competency to safely administer the prescribed home intravenous as per written and verbal instructions and physical demonstration. The Registered Nurse will be responsible for the teaching and supervision of the patient and/or caregiver. 6. The teaching process based on the patient and/or caregiver needs will include- written instructions, verbal explanations, demonstrations, evaluation and documentation of competency, proficiency in performing therapy, scope of physical activities and safe disposal of equipment. 7. Actions will be implemented prior to and during all home infusion therapy to minimize the risk of anaphylaxis/adverse reactions. The nurse will maintain the IV line and begin CPR as necessary while waiting emergency medical response. 8. The Registered Nurse will perform ongoing assessment of patient and/or caregiver(s) compliance in performing therapy related procedures at periodic intervals depending on the patient’s condition and the therapy, 9. The Patient and/or caregiver will be provided with a 24-hour access to the Registered Nurse during infusion therapy. 10.
