MDT Home Health Care Agency, Inc. ownership, hours of operation and scope of services provided Accessibility to MDT Home Health Care Agency, Inc. personnel after hours Patient Rights and Responsibilities Advance directives Homebound status Financial responsibility Emergency management Home safety Hazardous waste materials disposal Infection prevention and control Type and frequency/duration of services to be provided The Skilled Nurse, Therapist and/or Medical Social Worker provide instructions to the patient/family/caregiver before and during care, treatment and services and on discharge in strategies/skills/behavior that effectively address healthcare needs. These instructions may include, but are not limited to: Expanding the patient’s/family’s/caregiver’s knowledge and understanding of the skills and behaviors that promote recovery, maintain or improve function/the patient’s health, manage disease or symptom progression How and when to take medications in a safe and effective manner and any specific precautions related to their use Nutritional interventions, dietary requirements and any potential interactions with medications, and oral health Clearly informing patients/family members/caregivers in a manner and format understandable to them of their rights and responsibilities to actively participate, as appropriate, in the patient’s ongoing care, treatment and/or services particularly before the patient’s discharge from the Agency Instruction in habilitation and rehabilitation techniques to facilitate adaptation and/or functional independence Providing information about rehabilitation potential and resources The safe and effective use of equipment and supplies ordered for their use and provided by MDT Home Health Care Agency, Inc. Pain assessment process, effective pain management, and pain management strategies Storage, handling and access to medical gases and their supplies How to access other available community resources to meet the patient’s/family’s identified needs When to seek and how to obtain follow-up or continuing care, treatment or services as appropriate Providing an opportunity for the patient/family/caregiver to ask questions that allow them to make informed decisions concerning the patient’s care, treatment and/or services Promoting maximum achievable recovery and functioning The Performance Improvement Committee (QAPI) serves as one resource to review and revise as necessary organizationwide policies relating to patient education, to assure that Home Health Agency Nursing Manual J-46
