MDT Home Health Care Agency, Inc. PATIENT VISITS - SCHEDULING PURPOSE: To en sure that the care and/o services provided by the Agency staff are consistent with those identified throughout the care planning process to meet the patient’s needs. POLICY: Care and/or services provided by the Agency staff are consistent with assessment data. PROCEDURE:  The Registered Nurse/Licensed Therapist formulates a visit schedule based on the patient’s clinical status, needs and wishes at the time of the admission assessment and physician orders.  The frequency and type (i.e., discipline) of visits are communicated to the Clinical Supervisor who documents the information on a master/team schedule form and assigns appropriate personnel to provide care and/or services.  The Clinical Supervisor records all staffing assignments, including those for contracted personnel.  Each individual providing care and/or services to the patient should contact the Clinical Supervisor at least weekly to confirm that visits have been conducted as schedule.  The Clinical Supervisor should be contacted in a timely manner whenever changes need to be made as to the type, frequency, dates and/or times of patient visits.  The physician is contacted whenever changes to the type and/or frequency of patient visits are required based on patient needs.  Personnel failing to adhere to the visit reporting requirements may be subject to disciplinary action.  verification of completed visits includes comparison of submitted visit notes, time records and the master/team schedule.
