MDT Home Health Care Agency, Inc. The Registered Nurse/Therapist obtains a comprehensive medication history which should include the following information: Past sensitivities, including symptoms Past drug interactions, including those involving herbal remedies Past drug/food interactions Patient’s ability to comply with past medication regimens The Registered Nurse/Therapist performs an assessment of the patient’s current medications/herbal remedies to determine whether actual or potential interactions exist between the current medications and herbal remedies, based on the current body of knowledge. This information is documented in the visit notes of the appropriate discipline. Any questions or concerns about the patient’s current medications and herbal remedies are directed to the ordering physician. The list of current medications is incorporated into the plan of care. Copies of the plan of care is distributed/conveyed to all healthcare team members involved in the management of MDT Home Health Care Agency, Inc. patients’ medications, care, treatment and/or services, as appropriate. The patient/family/caregiver is instructed about the patient’s medications verbally and in written format understandable to the patient/family/caregiver. The patient’s/family’s/ caregiver’s response to instructions is documented in the medical record. The patient’s medications are reviewed and assessed during each skilled visit. Any changes to the medications or changes in the patient’s response to the medications are conveyed to the ordering physician. Home Health Agency Nursing Manual J-65
