MDT Home Health Care Agency, Inc. GOALS: 1. Improving the adherence to hand hygiene guidelines of our health care staff, thus reducing healthcare associated infection rates. 2. Be in compliance with our Infection Control Program: keeping hands clean prevents illness at home. Hand hygiene practices are key prevention measures in healthcare settings, including home care environment. 2. Increase patient and staff education: with more information at hand, our patients and staff will be better prepared, is our goal to continuously bring to our patient and staff educational material to help reach our goals in hand washing hygiene goals. 3. Prevent spread of infections: in healthcare settings, hand hygiene can prevent potentially fatal infections from spreading from patient to patient and from patient to healthcare worker and vice-versa. 4. The initial staff hand hygiene compliance goal setting by our Agency is at least 50% in the first 4 months of Hand Hygiene Improvement Plan implemented, with incremental goal of 2% monthly until we reach more than 75% compliance after the first 6 months, and is maintained thereafter. Actual Agency reached goal will be posted in the Agency’s Information Board for general staff information. PLAN: To achieve the Handwashing Goals our Agency has an Improvement Plan in place, In-service program annually that all field employee must participate, and annually in the field evaluation of their abilities to follow our Policy and CDC handwashing techniques (see Handwashing Evaluation Form), that must be signed by the Director of Nursing, Clinical Manager or Supervisor RN.
