MDT Home Health Care Agency, Inc. GOING TO DOCTOR'S OFFICE FOR APPOINTMENT 1. As soon as appointment is made: a. Note it in patient's chart. b. Call the agency to document. 2. Determine method of transportation: a. Family transport b. Handi-cab, requires 24 hour notice and immediate payment C. Ambulance, if immobile (large cast, etc.) 3. Determine if additional assistance is needed: a. Confused, noisy incontinent b. Request senior companion from the agency office 4. Furnish the following information: a. Correct name of patient b. Correct first and second name of doctor C. Correct address and phone number of doctor d. Correct time of appointment e. For the doctor, include: 1. New doctor's order sheet 2. New progress note 3. Copy of summary of orders for current month; and previous months, unsigned by doctor. 4. If patient goes to new doctor, include copy of front sheet (agency office will assist). 5. Do not send telephone orders. 5. Day prior to doctor's appointment, arrange transportation service the necessary transportation service for that patient.
