MDT Home Health Care Agency, Inc. GUIDELINES FOR MAINTENANCE OF PERIPHERAL IV SITES AND CENTRA LINES Heparin Loc: Site and injection CAP to be changed every 72 hrs./prn. Site care to be done every72 hrs./prn. Flush per SASH protocol: first with 1 cc N.S., give med, then reflush with 1 cc N.S. and 1 cc Loc Sol. (100u/ml). Even when not giving a med flush qd as stared above. Hickman Cathter: Site to be changed only by a physician. Injection cap change every week. Site care to be done 3xwk./prn. Flush per SASH protocol: first with 3-5cc N.S., give med, then reflush with 3-5 cc Hep Loc (100/ml) qd. When catheter is not being used, catheter can by flushed minimally 3/wk with 3 N.S. and 3-5cc Hep Loc Sol. (100u/ml). If you draw blood from the catheter, you must flush 20cc N.S., then3-5cc Hep Loc Sol. (100u/cc). Groshong Catheter: Site to be changed only by a physician. Injection cap change every week. Site care to be done 3xwk./prn. Flush per SASH protocol: first with 5cc N.S., give med, then reflush with 5cc N.S. only. The catheter does not require Hep Loc Sol. When catheter is not being used, catheter can be flush minimally 1.wk with 5cc N.S. If you draw blood from the catheter, you must flush with 20cc N. Triple Inmen CVP Catheters: Treat these catheters just like a Hickman catheter or Groshong Catheter. You must find out the type of catheter that was inserted. Exception: Must be flushed daily when not in use with 1-3cc N.S. and 1-3cc Hep Loc Sol. (100u/ml). Port-a-cath Pass Port: Site to be changed only by a physician. Huber needle extension tubing and injection cap to be changed every week and prn. Site care to be done 3xwk./prn. Flush per SASH protocol first with 5cc N.S., give med, then reflush with 5cc N.S. and 5ccHep loc sol. (100u/ml). When catheter is not being used, catheter can be flushed minimally once a month with 5cc N.S. and 5cc Hep Loc Solution (100u/ml). If you draw blood from the catheter, you must flush with 2occ N.S. and 5cc Hep. Loc Sol. (100u/ml). PICC Line: Site to be changed by a Registered Nurse only. Assess for type of Piccline, Hickman and Groshong. Treat this catheter just like a Hickman Catheter, or treat catheter like Groshong. An extension set luer lock can only be used and must be used. Exception: Must be flushed daily when not in use, with 3-5cc NS and 305 Hep Loc Sol. (100u/ml). Home Health Agency Nursing Care & Procedures K-107
