MDT Home Health Care Agency, Inc. JOB DESCRIPTION DIETITIAN/NUTRITIONIST The dietitian/nutritionist shall be currently licensed in this state, pursuant to Chapter 468, F.S., with at least 1 year of experience in dietetics and nutrition practice. Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager/Nurse Supervisor, Administrator. The responsibilities of the dietitian/nutritionist are: 1. To evaluate the nutrition needs of individuals in the home, using appropriate data to determine nutrient needs or status, and to make nutrition recommendations to the patient to maximize the patient’s health and well-being. 2. To provide dietetics and nutrition counseling in the home, as prescribed by a physician [ARNP, PA], acting within their scope of practice. 3. To observe and record activities and findings in the clinical record and report to the physician [ARNP, PA] the patient’s reaction to treatment and any changes in a patient’s condition. 4. To instruct the patient, caregiver(s), and other health team personnel in various phases of dietetic and nutrition treatment. Coordinating services with other disciplines as appropriate. 5. Ensure HIPAA guidelines and procedures are maintained. No physical requirement. 1. Able to speak, read and write in English. 2. Able read assignments, follow directions. This Job Classification will have a Potential risk for Occupational Exposure to Blood and other Potential Infections body fluids, protective equipment will be provided by our Agency to limit the exposure and will promote self protection practices in the delivery of the Home Health Care, to provide appropriate treatment to home health care workers in the event of exposure incident and to promote compliance with the universal precautions. MENTAL REQUIREMENTS: 1. Able to concentrate on detail with frequent interruptions. 2. Able to follow, complete and remember daily routines and requirements. 3. Able to comprehend and utilize professional education materials. 4.
