MDT Home Health Care Agency, Inc. QUALITY ASSURANCE/PERFORMANCE IMPROVEMENT JOB DESCRIPTION Reports To: Director of Nursing, Clinical Manager SUMMARY Under the general supervision of the Director, Audits patient records collecting and evaluating data, develops a plan of action for changes to produce quality care and positive outcomes for patients. (QAPI program) Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager, Administrator. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. · Audits patient's records collecting data for Quality Assurance (QAPI) on an ongoing basis for (OBQM) Outcome Based Quality Management and (OBQI) Outcome Based Quality Improvement. · Evaluates all data from the chart audits and develops a plan of action for Continuous Quality improvement. · Conducts Performance Improvement Committee Meetings for the Agency Staff. · Provides educational in-services for staff on the Performance Improvement outcome/issues. · Keeps current on OASIS, OBQM, OBQI, QAPI regulation and changes. · Submits quarterly reports to the organizations Director of Nursing, Clinical Manager. · Works independently and effectively with minimal supervision and is cost effective in use of time and supplies. · Completes all other duties as assigned. · Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct. · Complies with all applicable state and federal regulations. · Attends all mandatory meetings and programs. · Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times. · Maintains the confidentiality of patient, resident, employee and organizational information, HIPAA regulations. SUPERVISORY RESPONSIBILITIES Supervisory responsibilities as listed above. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
