MDT Home Health Care Agency, Inc. CLIENT’S CASE CONFERENCES POLICY Home health care staff members provide coordination of client services to established effective interchange, reporting, and coordinate client’s evaluation through client’s case conferences. PURPOSE To provide complementary services that support client’s plan of care objective. PROCEDURES 1. Client’s Case Conferences are conducted after the first 30 days of services, and more often as needed. 2. Participants in Client’s Case Conferences include, but are not limited to, all personnel providing services to clients, including: a. Director of Nursing, Clinical Manager b. Medical Consultant (if applicable) c. Nurse d. Therapist e. Home Health Aide 3. The coordinator of Client’s Case Conferences is the Director of Nursing, Clinical Manager of Home Health Care Agency 4. Subjects discussed in Client’s Case Conferences include, but are not limited to, the following: a. Client’s assessment b. Client’s intervention c. Client reassessment based on client’s intervention d. Development/implementation of client’s care plans covering: 1. Pertinent client’s diagnosis 2. Client’s mental status 3. Types of Home Health Care Services and equipment required 4. Frequency/duration of Home Health Care visits 5. Client’s prognosis 6. Client’s rehabilitation potential 7. Client’s functional limitations 8. Client’s activities permitted 9. Client’s nutritional requirements 10. Client’s medications and treatments 11. Safety measures to protect against client’s injury 12. Instructions for timely client’s discharge or referrals 13. Client vulnerability to abuse and/or neglect 14. Confidential and other pertinent items. e. Evaluation of client treatment plan f. Review of appropriateness of continued delivery of home health care services to the client. 5. The content and results of Case Conferences, are documented on a Case/Team Conference Form. 6. Appropriate documentation of Case Conferences establishing that effective interchange, reporting, and coordinated client evaluation have occurred, is made in the client’s clinical record.
