NUTRITION POLICY: It is our policy that clients are assessed for nutritional status and provided with nutritional intervention in conjunction with the client * s physician, when necessary. Clients are counseled about potential food interactions and specialty diets when necessary in conjunction with the pharmacist. PROCEDURE: 1. When necessary, the nurse reviews the client on admission and as changes in condition and needs arise. 2. Nutritional assessment is performed and potential nutritional deficits are identified. 3. The physician is notified of the potential for moderate to severe nutritional complications and if necessary, orders are received. 4. The client is educated about the need for adequate nutrition. 5. When necessary, the client and caregiver are educated in the proper storage and use of nutritional. Storage requirements include controlling lighting, ventilation, and humidity to prevent moisture. 6. Potential food interactions are reviewed with the cient/caregiver.
