MDT Home Health Care Agency, Inc. (d) Teaching, supervising and counseling the patient and family members regarding the nursing care needs and other related problems of the patient at home. 4. Evaluates the effectiveness of nursing intervention and updates the patient care plan as needed. 5. Communicates with the physician at least every two (2) weeks regarding patient's status, obtaining additional orders if necessary. 6. Is responsible for the care given by the Home Health Aide to assigned patients and does the following: (a) Supervises and evaluates the care given by the Home Health Aide every fourteen (14) days and more frequently if necessary, and submits completed forms. (b) Participates in periodic conferences with the Home Health Aide Coordinator concerning the Home Health Aides performance. 7. Maintains and submits written clinical and Agency records as deemed necessary to include: (a) Initial Assessment (b) Care Plan (c) Daily Notes (d) Discharge Summary (e) Change Orders (f) Recertification (g) Summary of Services (h) Weekly Schedules 8. Discusses and plans with the Director of Nursing, Clinical Manager at least weekly the following: (a) Next week's schedule including necessary frequency of visits. (b) Patient's general condition and required care and necessary changes in the P.0.T. 9. Communicates with other team members in order to provide comprehensive patient care. 10. Participates in committee activities and meetings. 11. Assumes responsibility for self-development by continually striving to improve his/her nursing knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading. Ensure HIPAA guidelines and procedures are maintained. JOB CONDITIONS/PHYSICAL REQUIREMENT The position requires the ability to drive extensively in a geographic area. Must be able to access patients' homes which will not be routinely wheelchair accessible. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care. There must be the ability to give and receive information orally. On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds. This Job Classification will have a Potential risk for Occupational Exposure to Blood and other Potential Infections body fluids, protective equipment will be provided by our Agency to limit the exposure and will promote self protection practices in the delivery of the Home Health Care, to provide appropriate treatment to home health care workers in the event of exposure incident and to promote compliance with the universal precautions.
