JOB DESCRIPTION TITLE OF POSITION: Community Health Pediatric Registered Nurse TITLE OF IMMEDIATE SUPERVISOR: Director of Nursing, Clinical Manager POSITION SUMMARY Nursing care is in accordance with the pediatric client's plan of care, and includes comprehensive health and psychosocial evaluation, monitoring of the client's condition, health promotion and prevention, coordination of services, teaching and training activities, liaison with physicians and provision of direct nursing care. Ensure HIPAA guidelines and procedures are maintained. RESPONSIBILITIES Functions: 1. Provides skilled nursing care to pediatric clients in their homes which includes: a. assessing the client's needs; b. initiating the plan of care and revising as necessary; c. providing those services requiring specialized pediatric nursing skills; d. consulting with and educating the client and family regarding the disease process, self-care techniques and prevention procedures; e. initiating appropriate preventive and rehabilitative nursing procedures; f. preparing clinical and clinical notes; g. coordination of services; h. analyzes and provides chart assessment for case conferences; i. referral to other services as needed; j. acts as liaison nurse with physicians, hospitals and specialty organizations and provides them with information and instruction; k. evaluating the effectiveness and outcomes of care; l. supervising LPNs/LVNs and paraprofessionals providing services to the client; m. planning for the client's discharge from service; and participating in inservice programs. OCCASIONAL DUTIES May be requested by Director of Nursing, Clinical Manager to fill in for the other district nurses when on vacation or sick. REQUIREMENTS Qualifications / Education: Graduation from an accredited School of Nursing. Recipient of a Baccalaureate Degree from an approved University program in Nursing preferred, but not required. Must be licensed in the State as a Registered Professional Nurse.
