JOB DESCRIPTION TITLE OF POSITION: Medical Social Work Assistant TITLE OF IMMEDIATE SUPERVISOR: Medical Social Worker Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager/MSW Supervisor, Administrator. POSITION SUMMARY: The medical social work assistant is responsible for the delivery of social services to the Agency patients and their families in accordance with a physician's written orders. These services are provided under the supervision of a medical social worker. Ensure HIPAA guidelines and procedures are maintained. REQUIREMENTS: 1. Baccalaureate degree in social work, psychology, sociology, or other field related to social work 2. One (1) year of social work experience in a health or welfare Agency 3. Licensure by the state, if applicable RESPONSIBILITIES: 1. Evaluation and assessment of the social and emotional factors related to the patient's health problem as a means of estimating the patient's capacity and potential to cope with the problems of daily living 2. Supportive casework provided for patients and their families geared toward acceptance of health problems 3. Provision of information concerning secondary effects of illness, especially emotional components 4. Acts as intermediary between patients and other community agencies and assists with nursing home placement 5. Assists patients and their families to locate and utilize community resources 6. Collaboration and consultation with the patient's physician and other health care team members to assist them in understanding the significant social and emotional factors related to the patient's health problems and in planning for discharge 7. Attendance at case conference meetings and staff inservice meetings 8. Prepares clinical/clinical notes on the day of the visit and incorporates same in the clinical record within 72 hours. Provides summaries and re-evaluations if indicated. 9. Planning and participation in staff development activities 10. Involvement in related community activities with responsibility for maintaining current information on available community resources 11. Participates in the development and/or revision of the physician's plan of treatment 12. Communicates effectively with all providing care 13.
