MDT Home Health Care Agency, Inc. a. Identifies self as the Home Health Care Registered Nurse to the client and client’s family upon arrival at the client’s home. b. Explains the home health care visit procedures to client and client’s family. c. Answers client and client’s family questions and concerns relative to Home Health Care Services. d. Requests the client’s signature on appropriate forms. e. Implements a comprehensive nursing assessment of the client. f. Develops and implements a client’s medical plan of care based on the initial nursing assessment and the attending physician’s, PA, ARNP, plan of care. g. Gives information regarding the client’s plan of care to the client and client’s family. h. Obtains and implements the physician’s orders i. Introduces an assigned Home Health Care staff member to the client and client’s family and their environment. j. Orients the client home health care staff members to the assignments and tasks. k. Informs the Director of Nursing, Clinical Manager of Home Health Care of staffing needs for the client and other patient’s information regarding the client. l. Discusses the Client’s Case Management in the first Client Case Conference following the client’s admission to home health care following Policy and Procedures: Client’s Case Conferences. m. Initiates/review a Client Sign Up Monitoring Checklist. n. Documents the following in client’s clinical records: 1. A comprehensive client’s nursing assessment on a Home Health Care Client’s Nursing Assessment form (See Nursing Assessment -OASIS). 2. The Patient Plan of Care and client’s outcomes in the client’s clinical record. 3. Implemented physician orders and client’s outcomes. 4. Appropriate information relative to the client 5. Home health care visits on a Home Health Care Visit Time Records form (See Home Heath Care Visit Time Records/Itinerary Form). o. Instructs the Administrative Assistant/Office Clerk to enter client’s information on a Record of Home Health Agency Client Visits form. 4. The Home Health Care Registered Nurse makes subsequent home health care visits at least once every two weeks (for skilled services). In these visits, the Registered Nurse: a. Identifies self as the Home Health Care Registered Nurse to the client and client’s family upon arrival at the client’s home. b. Answers client and client’s family questions and concerns relative to Home Health Care Services. c. Implements the nursing assessment of the client. d. Revises and implements the client’s plan of care bases on the nursing assessment and the attending physician’s, PA, or ARNP, medical plan of care. e. Gives information regarding the client’s plan of care to the client and client’s family. f. Obtains and implements physician orders g. Obtains the attending physician’s, PA, ARNP (Advanced Registered Nurse Practitioner), acting within their scope of practice, signature on the plan of care as appropriate. h. Assigns the clients to home health aides, as appropriate. Gives verbal/written instructions for the client’s care to home health aides, as appropriate. i.
