MDT Home Health Care Agency, Inc. Visit. j. Informs the Director of Nursing, Clinical Manager of Home Health Care of staffing needs for the client and other patient’s information regarding the client. k. Discusses the Client’s Case Conference following Policy and Procedures: Client’s Case Conferences. l. Initiates a Client Sign Up Monitoring Checklist and places it in the client’s clinical record. m. Documents for the following in client’s clinical records: 1. The nursing assessment 2. The revised and implemented patient plan of care and client’s outcomes. 3. The implemented physician orders and client’s outcomes 4. Appropriate information relative to the client 5. Home health care visit time, on the Home Health Care Visit Time Record form. 5. The Home Health Care Registered Nurse assigned to home health care on-call coverage makes the emergency home health care visits to clients, as necessary. In these visits, the Registered Nurse: a. Identifies self as the Home Health Care Registered Nurse to the client and client’s family upon arrival at the client’s home. b. Implements the nursing assessment of the client. c. Implements nursing interventions based on the nursing assessment of the client. d. Notifies the attending physician, PA, ARNP as appropriate e. Obtains and implements physician orders f. Assesses client’s outcomes. g. Upon physician orders, facilitates client referral or transfer to appropriate health care facility in light of the client’s changed health care needs. h. Answers client and client’s family questions and concerns relative to the emergency home health care visits. i. Documents the following in the client’s clinical records. 1. The nursing assessment 2. Implements nursing interventions and physician orders and client’s outcomes. 3. Appropriate information relative to the client. 4. Home health care visit time, on the Home Health Care Visit Time Record Form. 5. Outcomes of telephone calls in the Record of Agency ON-Calls Placed to Home Health Care Agency. j. Contacts on-call home health care administrative staff if concerns to problems arise that the on-call nurse is unable to handle. k. When the Home Health Care Agency office is open, informs the client’s Registered Nurse case manager or emergency home health care visits. L-10 Home Health Agency Client Assessments
